University MRI & Diagnostic Imaging Centers - Connect Provider Portal Agreement

PLEASE READ THE FOLLOWING AGREEMENT CAREFULLY BEFORE USING University MRI & Diagnostic Imaging Centers (University MRI) “Connect Provider Portal” [also known as “Erad Referring Portal”] service.

University MRI provides the website “Connect Provider Portal” (the "Site") and the personal health data viewing and reporting services (collectively, the "Service") subject to your compliance with the terms and conditions set forth in this agreement (the "Agreement").

1. Registration

In order to use the Services available on the Referring Provider Portal, you are required to Sign In to “Connect Provider Portal“ and be or become a registered user with an account. YOU ARE ENTIRELY RESPONSIBLE FOR MAINTAINING THE CONFIDENTIALITY OF YOUR USERNAME AND PASSWORD AND FOR ANY AND ALL ACTIVITIES THAT ARE CONDUCTED THROUGH THE “CONNECT PROVIDER PORTAL”.

2. Code of Conduct

While using the Site and the Service, you agree not to:

  • Restrict or inhibit any other visitor or member from using the Site.
  • Use the Site or Service for any unlawful purpose.
  • Express or imply that any statements you make are endorsed by us without consent.
  • Transmit:
    • Any unlawful, fraudulent, threatening, abusive, libelous, defamatory, obscene, or objectionable content.
    • Non-public company information without authorization.
    • Any third-party trade secret.
    • Unsolicited commercial communication.
  • Engage in spamming or flooding.
  • Transmit any malicious software (viruses, worms, trojans, etc.).
  • Modify, reverse engineer, decompile, or disassemble any part of the Site or Service.
  • Remove copyright, trademark, or proprietary rights notices.
  • Frame or mirror any part of the Site without authorization.
  • Use any automated process to retrieve or reproduce Site content.
  • Collect information about Site visitors or members without their consent.

Any changes we make will be effective immediately upon notice, which we will provide by posting on the Site. Your continued use of the Service after such changes will be deemed acceptance of them.

Your use of the Site or the Service, and any dispute related to use of information provided to us, is subject to the then-current version of the Terms of Use agreement. IF YOU DO NOT AGREE TO THESE TERMS, PLEASE DO NOT USE THIS SITE OR THE SERVICE.
